SPEAKER_04Elliot, Elliot's good. All right. So can um for my audience, can you do me a favor and just introduce yourself, uh, give yourself a little bit of a background on what you have going on, and of course, we'll get into something like technology. And and as you can see, if you guys are watching here on my page, you're the founder of FirmTech, so we can get at that in a little bit.
SPEAKER_00Sure. Well, by deep background, I wanted to be a spy many years ago when I majored in Slavic language and literature in college, uh, and then took the test for the CIA. I didn't like those people. So I decided to become a doctor. That was my backup plan.
SPEAKER_04Wow. Many many years.
SPEAKER_00So uh, you know, and I spent many, many years in emergency medicine. And, you know, as an emergency medicine doctor, there are a few emergencies that concern a man should have a heart attack or a stroke as much as a limp dick, especially if it goes on for two or three nights in a row. Uh and then uh emergency medicine led to some interest in in telehealth. I founded a company called Swift MD, doesn't exist anymore, but I founded around the same time as Teledoc. It was the second oldest telehealth company in the country, and that got me interested in remote patient management technologies. What type of technology can people have at home that would make them smarter than their doctors? So they ideally would not have to see a doctor.
SPEAKER_05Right.
From Neuromodulation Experiments To A New Idea
SPEAKER_00And I really uh I mean that's things are starting to flourish, you know, after you know, after after COVID when the government really opened up to telehealth. Uh I I live in Montana um and I ride horses. My wife says I ride like a 12-year-old. I'd like to feel say I'd rather more like a 25-year-old, but I tend to be fairly, let's just say, borderline reckless in some people's minds. So uh I um someone had cut a trail, cut a rather cut a tree that was hanging over a trail because you'd walk underneath it, but you couldn't ride a horse underneath it when you're going 35 miles per hour. So uh so I I like to feel I shared that tree with my chest, but I did break six ribs and six vertebrae. Uh and fortunately, my my spinal cord was not injured, so that guy'd be interested. Well, what's been done to rehabilitate sexual function of men and women who have suffered a spinal cord injury or spinal cord disease, right? Or frankly, or have or have some significant rectile dysfunction. So they could do some some help. Right. So we're all familiar with cardiac pacemaking, which is implanting electrode by the heart in order to control the heart with a genetic path, usually buried under the skin. Uh and that's called neuromodulation. So there are several papers in the medical literature uh claiming that they were able to more neuromodulate erection, erection and even ejaculation in men. Uh, men with spine men with multiple sclerosis or spinal cord injuries. Uh, and by the way, whenever I hear the phrase medical literature, I think most of it probably is fiction, you know, it's like literature. Especially after COVID, hopefully people have learned that lesson. And then now quite you know, several, you know, enormous exposes showing that the majority of research, frankly, is not reproducible and was not reproducible because of misconduct, or just that's just you know, it has a studied larger numbers, you know, it didn't the evidence didn't pan out. Anyway, yeah, with these papers. So I um I first I tried out on two rams, male sheep, uh, and we got erection and ejaculation, but we also got defecation and urination. So it wasn't exactly bedroom-friendly. Um and I then had a choice to make. I could, you know, go to out to the valley or to your city or someplace to try to raise 15-20 million dollars to continue this experiment on people in Nigeria or uh Brazil who had who have significant erectile dysfunction, or I could test it on myself. So I uh had a friend I have a friend who was a urology surgeon uh and went to his clinic in San Francisco, San Jose, uh, and he implanted electrode by microdendal and cavernous nerves, everyone's two favorite nerves that no one has heard of, but they're the nerves that are responsible for arousal and orgasm in men and women. They come out just me underneath the pubic bone. Uh and I felt nothing, but you know, regardless of the amplitude, frequency, or the stimulation, I felt nothing other than tingling. So I don't actually that that that was really interesting because it raises the question. I mean, everything in the body ultimately comes down to electricity. And it raises the question of uh well, what is really producing erection arousal? And are those the right nerves? Well, there aren't that many other main nerves down there. So what what is really going on? And we don't I don't think I don't think we know um a lot about about just how erections are produced uh and how orgasms occur. I think we we have some notions about the sympathetic and the passive in the nervous system.
SPEAKER_02Yes, I've heard a lot about that.
Nocturnal Erections As A Cardiovascular Signal
SPEAKER_00But um didn't work controlling at the level of one I guess just one person, me, but controlling it at the level of the nerves that enter the penis by the pute bone, didn't feel anything. Anyway, I called that project O for obvious reasons, uh and uh tried to get that published, but negative studies, especially negative reports in the urology literature by someone off from outside of the field on emergency medicine doctor, they don't get published. But I was the late so a few years passed, and I was working on some other projects involving sensor technologies. I was working with I was working with a surgeon at the University of Utah to try to um control blood pressure after stroke by putting a catheter up into the up at the arteries of the base of the skull, the circle of wills. Uh and he a last day of ski season, this is like four years ago, he was speaking to uh maybe four and a half years ago, he was speaking to uh a professor of urology at the University of Utah and said, Hey, you know what? You're a urologist. I have this funny story to tell you about this doctor I'm breaking with in Project O, and that urologist uh urologist named James O'Tal and he's now my chief alcohol officer, said, Wow, well, I would like to come up with a way of counting the number of not nocturnal erections in man. Let me speak to him. So he contacted me, and when I when I said when he told me we want to count the number of nocturnal erections, I thought, why? I mean, you know about morning wood and you laugh about it, but what but he said that the penis is the canary and the coal mine of cardiovascular disease, and one erection is not enough. I mean, you can still function if you have one, you know, if you have morning wood, but it doesn't necessarily mean that you're in sexual or good cardiovascular health. And a healthy man should have a healthy man should have three to five nocturnal erections per night, and that took kind of took me aback because uh we don't have a lot of leading indicators in medicine, right? We treat associations of medicine, high blood pressure associated with stroke, health proteins associated with atherosclerosis, high blood pressure associated with heart attacks, so is diabetes, but we don't but they're not predictive. I mean, if you're I just make up a number, you know. Um, Carlos, if your blood pressure went from you know 120 over 80 to 140 over 90, it's not telling you that you're gonna have a stroke, just saying, hey, here's something you should pay attention to, right? As a trend. Uh, and whereas if a man's nocturnal erections go down by 50%, especially below one nocturnal erectional less per night, that man has a close to 50% chance of having a cardiovascular problem, could be a heart attack, adjust something over the next two or three years. Well, that is profound. And the and yeah, like half the men who have heart attacks retrospectively report um significant retinal dysfunction six months preceding it. I think it's like 80 or 90 percent of the men with strokes. So following nocturnal erections we would be really valuable in terms of cardiovascular health, but also in terms of sexual health as well, too. So that got me excited, and that's how that's the backstory of how we we got the project.
SPEAKER_04So let me ask you a couple questions. So I guess then if uh if you're having a good amount of erections per per night, that is a good sign, you would say, of having good cardiovascular um because of the amount of erections you're getting.
SPEAKER_00That's right. Well, you might still have a problem, but it's a lot of correct problem, it's a lot less likely.
SPEAKER_04Right, okay, got you. That that's that's really interesting because I was wondering, you know, and this is again in my channel, I'll share a lot of things, but uh recently um I've I've told you uh the conversation we had the other day. I've suffered from chronic prosthetitis and bacterial prosthetitis, right? But after all those issues, you know, and they have gotten better, I've noticed that I constantly get a lot of erections at night and very often.
SPEAKER_03And I'm not sure if I should be uh worried or I should be I should be happy. I wouldn't worry about the calls.
SPEAKER_00How calls, how old are you? I have 32. So you should have like five a night. Uh, I don't I wouldn't worry about that at all. It's probably it's it's good.
SPEAKER_01Oh normal normal, yeah.
Tech Ring Overview And Data You Get
SPEAKER_00Okay, cool. Gotcha, got you. So now if if if they're you know uh yeah, I I I I wouldn't worry about that at all. It's good.
SPEAKER_04Yeah, okay, no, got you. So so then what um what technology did you or are you working on or did you create that helps men with the erectile dysfunction?
SPEAKER_00Sure. Well, what but we basically have done is embedded sensors into an erection ring. I don't like calling them cock rings because that puts it in the realm of a of a novelty, something you you get for a couple times a year at a sex toy store and you masturbate a few times or have sex a few times and you get rid of it. And why is that? I heard of that before, yeah. Yeah, because they're these essentially they're an O-ring. Um, and then they're hard silicone, they pinch, they're uncomfortable, can't be really worn safely for more than 20 or 30 minutes. Can you see my page real quick?
SPEAKER_04Uh what you're referring to, right?
SPEAKER_00Yep, that's that's the tech ring, and that's the you know, we have easy to use uh Bluetooth app. We provide people with data about their their number nocturnal erections, how long they last, how hard they are, and uh their sex erections, how how long they last, how hard they are. And with this data, men can then measure the impact of diseases, diabetes, hypertension, athosclerosis, um impact of medications. Right. Do I need to if I'm taking testosterone, what's the right dose? So I'm gonna take a PD5 medication like for agrocyalism. Do I even need it? Or what's what's the right dose? Uh you can measure the impact of alcohol, recreational drugs, relationships, this ring can apply on your sexual performance because this ring can be worn not just overnight to get data, but it can be worn during sex as well. Because it also functions in order to come up with a ring. I got into this with data, then realized I I had both with the data we discovered that the leading cause of erectile dysfunction is not what the doctors think it is. As a matter of fact, most men who have VD don't have ED, they just got a problem. They don't um and we also so in order to come up with a ring that we won overnight, we have to make a ring that's out of it's made out of soft silicone, soft um saltelastomer, it doesn't pinch, it allows the arterial blood to flow in, it only constrains the venous return if you have an erection, otherwise, it doesn't interfere with the flow of blood at all in any significant way. So, but that means we also invented a safe erection ring that could be used for for pleasure as well, too. So the data we and we now have documented 137,000 plus erections on close to 4,000 men. Wow, yeah, and this is the largest dick database in the world. Uh and and and pre um and previously um uh the understanding or or you know what doctors were thinking was going on with men was based upon the study of like 550 men in the 1990s using device called the Ridge scan. It looks like something Dr. Frankenstein put in your dick. It can't be used during sex, and I just let these these loops attached to your penis go into a box, uh, and you have to go to sleep lab most of the time to get this done. Oh, that must be uncomfortable. It just fixed it, it fell out of fashion. We had to like lie there on your back, and it was you know, it was it it was it wasn't practical. Um it was 1990s technology. We we're we're back we're in the 21st century with yeah, hey, you have to have an app, right? And have we have to provide we have to provide notifications and uh you can compare yourself, you know. So, for example, I mean you're at the young end, the vast majority of the men who use a device are between the ages of 45 and 65. And if you're in that age group, uh you're getting and you use the technology, you're comparing yourself to because we we tell men where they are in comparison to men five years older than them, five years five years younger than them. That that's that's that's their you know their set. You know, if if you're you know so you're comparing yourself to a couple thousand men, so you can really see where you where you stand, and but then of course, you don't want to be average, right?
The Real Cause Of Fading Erections
SPEAKER_04You want that's so interesting. You want to be optimal, you want to be the top 10 percent. You want to be that top dog. So so the the science behind it then, because my understanding was I thought blood flow was the the reason why your penis gets erect.
SPEAKER_00What's the science behind it? It's about blood flow. It is about blood flow. Yeah, so so the um you know the academic urologist who set the standards with what were seeing the sick the sickest people in their practice, they're seeing the men who struggle to get erection or can't get erection at all. Okay, those men are far more likely than not have a significant cardiovascular problem. The heart's pumping, the heart's no longer pumping enough blood. They got angina, they've had a heart attack, they have congestive blocca, there's something wrong with the pump. Uh, then they often, in combination with that, have a problem with the pipes, the arteries that come out of the heart that go in, you know, that go to the penis, diabetes, diabetes, hypertension, naphosclerosis can narrow those arteries. So often you have both combination of pump failure and poor pipes, narrow pipes coming out, and do you have a penis that that uh instead of getting five erections per night gets two erections per night instead of average erection lasting 20 to 35 minutes? The average erection might last five or ten minutes. Uh and that's that's a problem. But that's only about 12 to 12 to 13 percent of men. So the technology is where our technology is valuable because it will identify who those men are because they will not have uh average or above average you know performance in terms of duration, firmness, and monoctural erections.

SPEAKER_00But the vast majority of men have have have another vascular problem, which is which is doctors call it venous leak. I call it the fading erection of aging, well, it can be multifactorial. But all men, all men have have erections that fade faster as they get older. Carlos, when you were 13 years old, the girl could hold your hand for five minutes and peck your cheek and you get a heart on for an hour. Uh, when was when was the last time it happened to you? A long time ago. A long time ago, exactly. So blood, you know, blood, blood leaves your blood doesn't butt empty, but blood leaves your pants a lot faster, you know, right now than it did then. Any any guy any guy can so men get erections and they lose them. And they could lose them for a whole variety of reasons. It could be hey, your your wife's cheating on you, or maybe you're cheating on your wife, or you've had too much to drink, or you're worried about paying the mortgage. Uh there's a problem with a kid.
SPEAKER_04Lymptic as they call it, right? When you drink too much and whiskey, whiskey dick, yeah. Whiskey dick, yeah.
SPEAKER_00But but commonly guys that guys with they get an erection and then they they can't they can't have an orgasm or they just or just it just fades faster. Right. Um, or it could be that you're taking medications that we doctors prescribe that have an impact on your sexual health without us warning you about it. There are anti-hypertensors that will do that, right? They're anti-anxiety medicines, antidepressants. Um, there are medications for prostate problems, flow medicines as well, like flow nights will also interfere with your ability to sustain erections, and or it could be a combination of these various factors. But if you're a man who gets an erection, then loses it. The PD5 medications for agrocyalism being the most popular ones, they put more blood in, and that's good. But the major challenge is keeping the blood in, and the therapy should be correct, keeping the blood in, and the answer is a ring. Because what does a ring do? A ring compresses our rings being of course the best one, but anyone will do, but a ring, but a ring, but a ring, um, you know, come they they um compress the superficial veins and hold the blood in the peas, and men need to open up their minds to it. So we shouldn't be using the word ed for most for most men, because for the guys who can't who struggle to get erection or can't obtain erection, that's dysfunction. And those men see a cardiologist and/or a urologist to try to get that problem right out. But for the vast majority of men who get an erection then lose it, well that's a different problem. That's a different problem, and that's a problem on the venous side of the circulation. And the answer for those most most of those men is a is a ring. Now, there are men who have venous leaf that's so severe, it doesn't work. But for the vast majority of men get erection and lose it, the ring is is effective. And I didn't I didn't I didn't understand this.

Rings As Treatment And Performance Optimizer
SPEAKER_00Four years ago, I would four years ago, one that was no data. Uh and four years ago, I would have been like most guys, I would have said, Well, I don't need it. So let's talk about the data first and then the need. So with this large database we have right now, uh the expectation was as men get older and the incidents and men report more erectile dysfunction. Right. The the and we the incidence, the number of nocturnal erectile come down, the firm's nocturnal rectal go down, and the firm's sexual go down. Well, that's true for the 10, 12, 10, 15% of men who have severe significant cardiovascular disease, but most men, as they get older, they got a little, they got borderline hypertension, they might have borderline diabetes. They just start softening up a bit until in the late 60s and early 70s, and not enough that they should say, hey, I my penis doesn't work. Well, uh the problem they have they have is that their rectangles fade faster, and they need a tool to help optimize it. Women are vibrators, men should use rings. Because all men, whether they have ED or don't have ED, benefit from the optimization that occurs the occurs of the ring. Uh here are five questions that every man should ask himself. You too, Carlos, you're a young guy, you probably never use an erection ring in your life. Or if you did, or if you did just play around once or twice. Here are five questions, Carlos. Uh I'm I'm gonna I'm gonna pretend right now that's that you're your typical uh heterosexual doctor at a trade show. Hey Carlos, did you use erection rings? Nope. Nope. And then would do you need think you need one? I don't feel like I do at the moment. Hey, well, Carlos, it's not about need, it's about what. Answer these five questions, Carlos. Would you like to rock harder? Because beyond beyond cock beyond beyond rock beyond beyond masturbation or whatever or sex, there's there's cocking hard. Nothing will get no amount of manual oral batch simulation is gonna get hard. So because you want to rock harder, good. We're making some progress here. Um, do you want to last longer? Sure. Sure. Would you like to have more a more intense orgasm?
SPEAKER_03I've never had that, but that sounds interesting.
SPEAKER_00Yeah, because right now I'm guessing things are good, but what if things could really be great? Because our ring is and uh we designed our ring specifically to put the right amount of pressure over the urethra to draw out the ejaculatory phase by 50 percent. So my eye phase our ring goes from four seconds to seven seconds. Well, that's an incredible orgasm. But let me ask you another question. So uh, so um, would you like to be more confident? Would you like to feel if you're with someone, especially the first or second time, that if you're nervous, once you get hard, you're gonna stay hard. Would that be good? Yes, yeah, and what about what if you what if you could have that third cocktail not to worry about or fourth cocktail, not to worry about performance? Would that be good? Yeah, that would be and what if you can what if you can stay up till like one o'clock in the morning, spend an extra hour dancing, and even though you're kind of tired, know that you're still gonna be good. That's good. Yeah, that's good. Okay, now would now would you like your would you like your partners to be more confident than you? Yes, yeah, we'll put a ring on it, dude. Because those those are five, those are the five questions that every man should ask themselves. It's not about need needing something, it's about wanting it. Um, but then there's the data. So let's suppose, you know, so now Carlos, if you were like to pretend, if you were if your guy was like 50 years old, right? You should want your data. You should want to know the data whether even if it's normal, because when we go to the doctor when we're 45, 50, we don't tell the doctor, I don't want any tests because everything's gonna be normal. No, we want the test because the test is gonna establish a baseline, even if our little proteins are normal, our blood pressure is normal, but carnival, at least now we have a baseline, and that's every man should get should know their baseline, their sexual health. That's quite a bad go ahead.
Comfort, Safety, And Partner Experience
SPEAKER_04Doctor, quick question. Looking at this, right? And I'm thinking about the ring, and I'm gonna put the picture up here again so um we can both see it. So when you look at this ring, how comfortable is this? Like uh, how is it you know I you know I respect you when you're you know when the you're penetrating?
SPEAKER_00No, no, it'll be it's it's a soft elastomer. Um, and uh it's about three-eighths of an inch in in width. No one look, the truth is only in only in in TV movies do women like being banged banged really hard to the bone. No one likes that in reality. So if if you're if if your penetration was three-eighths of an inch less, you think anyone's gonna notice? By the by the way, there are no there are virtually no nerve endings inside the vagina anyway. The nerve endings are on the outside, around the outside of the vagina.
SPEAKER_05So whether you the length, the length the length the length is an aesthetic thing.
SPEAKER_00Hey, you know, some some people are turned on by more length, and dollars, you know right. Okay, so the reality is um this is not you know, we don't and we have women who like it because if they're on top, then now something soft to grind against with a pull of a lube on that soft elastomer. Oh, that makes sense, as opposed to grinding clitoris on hard pubic bone or pubic hair. So then now let's talk about the male comfort. The male, the male comfort is really enters into it if it's being worn over at night. Uh, eight to ten percent of men for the first two or three times they wear it, complain about it feels uncomfortable. Well, got look guys, we're not we're not we're not putting things in our dick over at night, you know, women women go to bed often wearing underwear and stuff, and then you know, men don't. Um uh it's you know, just you just get used to it. I encourage these guys, if they say that, you know, to well for one thing, if it's gonna pull on your pubic hair, pull your pubic uh cut and pull the pubic hair out of the way.
SPEAKER_05Right.
SPEAKER_00Uh if if uh you know, um if you probably have noticed uh that women eight to ten well, the average woman eight to ten times a day adjusts her bra. Why is that? Because it's a little bit uncomfortable because the elastomers or silicone is pulling it, is pulling into the skin. Well, it's the same thing with with this ring of this water overnight. You just if it's pulling in your skin, you just unhook it, rehook it, reposition, go back to bed. Well, you just pull in a loop and delet it settle into a different part of your skin, then you go back to bed. So it's it's statistically, uh, eight to ten percent of men take a few times nights to get to get right.
SPEAKER_04Is there is there with this, does this help pre-ejaculation at all? Is there a correlation with premature ejaculation?
Premature Ejaculation, Orgasm Duration, Refractory Time
SPEAKER_00Yeah, well premature ejaculation, yes. Yeah, it helps afterwards because um the uh you want to stay higher, you know, guys with premature ejaculation want to stay higher longer. So if you with um so we did we did two studies of D2 messes. D2 mess is the time it takes for an erection to go down after a climax, right? With nothing, it's 25 to 35 seconds, but with the ring, it's two to three minutes.
SPEAKER_02Oh, it takes a lot, it takes a lot.
SPEAKER_00It's gonna take it's gonna you're gonna be hard for two or three minutes. Well, that's you know, you've got pretty much your ejaculation, that's a good thing. At my age, it's a romantic thing. Because at my age, I I don't, you know, I'm married for 37 years.
